Folk College 2012 Registration
Please fill out a separate form for each person!
Two ways to register:
1. Print out attached pdf file and mail.
2. Fill out and submit the form below and mail.
3. Mail registration to: HCAC, PO Box 216, Huntingdon, PA 16652.
Registrants under 18 must be accompanied by a parent or guardian.
Registration confirmation will be sent via e-mail.
Refunds can be given only for cancellations before May 1, 2012.
